Is an MCA only for struggling businesses?

No. Depending on the industry and how much time a business has to manage a more traditional facility, an MCA can be a quick, easy way to access capital. The trade-off is cost: these facilities are usually more expensive than a traditional overdraft or line of credit, which is exactly what the numbers above are showing you. Asset security is rarely, if ever, required for an MCA. Repayment comes directly from a percentage of your daily card sales instead.
They’re the same thing described two ways. The holdback rate is the percentage of daily card sales taken toward repayment; the factor rate sets the total amount owed.
